Breast Reconstruction will not be covered by your insurance benefits unless it is deemed medically necessary. All decisions are made on a per-patient basis. If the surgery is deemed medically necessary, certain PPO plans that include out-of-network benefit options for specialists or surgeons may be used. In those cases, it is important to note that the related fees (for anesthesia and the use of the surgical center) may or may not be covered by the insurance.
